PAYMENT POLICY
Payment plans are available. Arrangements for payment must be made at the time of registration. Payments may be made by cash, Visa, MasterCard, money order, or checks-7 days before the class start date. Course fees final payment due prior to 1st clinical.

CLINICAL ATTENDANCE IS MANDATORY – Clinical attendance is mandatory. Any/all absences will result in a $280.00 FEE. All clinical absences must have documentation to be excused, although a fee will be assessed. A No-Call, no –Show is grounds for immediate termination from the program without refund. Additionally, all clinical hours must be met as stated in the course outline. All students are required to have a TB test and statement of good health before the first day of clinical.

STUDENT PROGRESS
Students shall be informed of the standards for the course as well as the basis for judgment in grading. Student progress is continuously recorded on a Grade Unit Tracking Sheet, which records achievement (by percentage) in the subject matter for each section of the course. The course workbook must be completed in its entirety for class credit. A grade point average of 77% (77% for Basic Nursing Assistant) is required to pass the course. Final grading is on a PASS – FAIL basis. Please see specific course requirements for any additional information. Students may address any issues, including academia,

Make-up
Make-up work is the sole responsibility of the student.

CLASS DECORUM
Smoking is prohibited inside the school facility.

ATTENDANCE AND CONDUCT POLICY
Attendance- Regular attendance is mandatory for course completion.
Absenteeism – In the event of unexcused absenteeism of more than 10% of the course, termination of enrollment will result. In the event of excused absenteeism, make-up time may be granted. Excused absenteeism may include but is not limited to the following: hospitalization, illness needing medical care, or death in the family. To obtain an excused absence, the Nu Era Training Academy office must be notified on the day of absence.

The office number to report an absence is +1 (708) 858-2158. Additionally, students must provide documented proof stating the reason for the absence. Make-up time may be granted for an excused absence upon approval of the director. Make-up time must be completed within 50 days of the absence.

TARDINESS – Students are required to be on time for all scheduled classes. Excessive delay is defined as being late to a scheduled class more than 10% of the scheduled class sessions. In the event of excessive unexcused tardiness, consultation with the Director of Nu Era Training Academy may be required.

LEAVE of ABSENCE – Leave of absence is defined as more than a 3 consecutive day absence from the Certified Nursing Assistant Course. To be considered for a leave of absence, students must complete a Leave of Absence Form and submit it to the Director of Nu Era Training Academy Leave of Absence Forms are available in the office. Under no circumstances will any Leave of Absence exceed 3 months from the last day of attendance. All relevant classroom material must be returned to Nu Era Training Academy, and outstanding balances must be paid before a leave of absence will be considered. If a leave of absence is granted, it is the student’s responsibility to contact the Director at least two weeks prior to re-entry into the approved class. A failure to return on the assigned re-entry is an automatic termination of the student’s enrollment.
Student Conduct and Discipline – Any student at Nu Era Training Academy who engages in any of the following will have his/her enrollment terminated:

Cheating or Plagiarism
• Furnishing false information to school officials
• Physical or verbal abuse of another person on school premises
• Theft or malicious damage, or destruction of property at the instruction site
• Lewd or indecent conduct on school premises
• Possession, consumption, or under the influence of alcoholic beverages or illegal drugs on school premises.
• Disorderly conduct
• Failure to comply with proper and lawful direction from an official of Inc.
• Use of firearms, dangerous weapons or other dangerous devised on school premises
• Unauthorized entry to school facilities
• Violation of local, state, or federal laws while on school premises

PLACEMENT POLICY
Career guidance and placement assistance are indispensable responsibilities of any successful career-oriented training program. Nu Era Training Academy guidance and placement policy consist of these main areas:
• Assistance in creating a resume, developing good interviewing skills
• Maintaining a job lead board available to all students

The student has the right to cancel the initial enrollment agreement until (midnight) of the (5th) business day after the student has been admitted. If the right to cancel is not given to any prospective student when the agreement is signed, then the student has the right to cancel the agreement at any time and receive a refund on all monies paid to date within (7) days of cancellation. Cancellation should be submitted to the authorized official of the school in writing.

Cancellation of the enrollment by midnight of the 5th business day after this contract is signed by the student and accepted by Nu Era Training Academy.

Refund Policy
To avoid misunderstanding and possible confusion, all requests either from the perspective and/or current student or the school, for withdrawal/dismissal and possible refund of monies paid should be made in writing. The accepted means under which this is done is through certified mail.
Cancellation of the enrollment within 5 business days by midnight of the 5th business day, after all parties have signed the final agreement, will result in the total refund of the tuition/fees paid to the school.
A. When notice of cancellation is given before midnight of the 5th business day after the enrollment, but prior to the 1st day of class, all tuition and any other charges

shall be refunded to the student, but prior to the student’s completion of 5% of the course instruction.
B. Nu Era Training Academy may retain an amount computed prorate by days in class plus 10% of tuition and other instructional charges up to the completion of 60% of the course. The school will retain the application/registration fee and the entire tuition and other charges. When notice of cancellation is given after the student has completed over 60% of the course, measured by days in class, Nu Era Training Academy will retain the application/registration fee and the entire tuition and other charges.
1. A student who, or personal initiative and without solicitation enrolls, starts, and completes a course of instruction before the 7th calendar day after the enrollment is signed, is not subject to the refund provisions of this section.
Applicants not accepted by Nu Era Training Academy shall receive a refund on all tuition and fees paid.
4. Application/registration fees shall be $75.00 unless otherwise specified in writing. At no time will this fee exceed $150.00. The registration fee is nonrefundable.
5. All deposits of down payments shall become part of the tuition and/or fees.
6. Nu Era Training Academy will provide a copy of the student’s valid written enrollment agreement once accepted.
7. If Nu Era Training Academy cancels or discontinues a course, the student shall have all tuitions fees returned.
8. All student refunds shall be made by Nu Era Training Academy within calendar days from the date of receipt of the student’s cancellation.
9. A student may give notice of cancellation to Nu Era Training Academy in writing. The unexplained absence of a student from Nu Era Training Academy for more than 15 school days shall constitute constructive notice of cancellation to Nu Era Training Academy for the purposes of cancellation; the date shall be the last day of attendance.

10. Nu Era Training Academy reserves the right to make refunds that exceed those prescribed in this section.
11. Nu Era Training Academy shall refund all monies paid to it in any of the following circumstances:
A. Nu Era Training Academy did not provide the prospective student with a copy of the student’s valid enrollment agreement. Nu Era Training Academy cancels or discontinues the course of instruction in which the student has enrolled.
Complaints
Students may file a complaint with the institution in writing by emailing the (Program Coordinator) Dr. Sunya Watson [email protected]. Students may also schedule appointments to speak with instructors and the program coordinator by emailing [email protected].
All complaints will be reviewed and addressed within 72 business hours. Students can visit with instructors/program coordinators during office hours and schedule an appointment via email.

Complaints against this school may be registered with the Illinois Board of Higher Education by sending a letter to either of the following addresses:
